Solved my problem - thanks! That happens whether they have User Profiles or not. When a user is deleted in AD their Profile is removed, but their entry in the user list is left in place for history. This video shows you how. It's worth noting that in my case I couldn't view workflow settings across my whole site collection and it wasn't just 1 sub-site affected. If there are no workflows there, I can forward this information onto our Operations team. Template images by Raycat.

So in the end, the problem was due to the "Workflow" feature at the site collection level not being right.
